Add -tls-proxy-outbound-port flag to jafar (useful for non-HTTPS protocols)

Edit README to make it explicit that tlsproxy has nothing to do with HTTP and can be
used with any TCP protocol that does TLS handshakes.

There's no point in doing that. Also, once this change is merged, it becomes easier to cleanup/simplify netx.

* refactor: move more commands to internal/cmd
Part of https://github.com/ooni/probe/issues/1335 .
We would like all commands to be at the same level of engine
rather than inside engine (now that we can do it).
* fix: update .gitignore
* refactor: also move jafar outside engine
